Life After Stroke Digital Pathway

Background

NHS Lothian and NHS Fife has been in partnership with Chest Heart and Stroke Scotland (CHSS) and Pogo Digital Healthcare to develop a new pathway for Pogo’s Tailored Talks digital platform. The new Life After Stroke service aims to put the Tailored Talks digital platform at the forefront of the service, ensuring tailored stroke information is available to those who need it, including to patients directly as well as family members, for as long as required after discharge from hospital.

A pilot, due to launch in June 2024, will offer stroke survivors to be onboarded onto the Tailored Talks digital platform, allowing individuals to input their symptoms to monitor longitudinally, and to access information directly relevant to their managed symptoms.

Aims & Deliverables

  • Improve access to digital high quality, tailored information on self-management for patients.
  • Improve patients’ understanding of their condition and common symptoms and issues after discharge.
  • Provide patients with a way to monitor their progress with their recovery after stroke.
  • Provide clinicians with a reliable method of getting relevant information to patients without increasing their administrative load.
  • Provide valuable data on patients’ progress with their stroke recovery.

Expected Outputs

The pilot will initially launch within the Royal Infirmary of Edinburgh, with patients who are eligible for digital platform to be offered registration via healthcare professionals. We expect to expand the offering to patients within Western General Hospital and St John’s Hospital once an initial evaluation of the pilot has occurred. Eligible patients can additionally be offered access to the Life After Stroke digital platform through CHSS nurses.
